Springside Nursing and Rehabilitation is seeking a Per-Diem Licensed Social Worker (LSW) to join our fantastic team! Requirements for Licensed Social Worker (LSW):
-    Must possess a minimum of a B.A in Social Work, MSW preferred
-    Active Social Work Licensure in Massachusetts, in good standing (LSW, LCSW acceptable; LICSW preferred)
-    1+ years skilled nursing facility experience, preferred
-    Required:  A strong desire to support and guide residents and their families through their later years with compassion and dignity.
Job Duties of Licensed Social Worker:
-          Complies social history data
-          Participates in assessments, development and evolution of residents care plan.
-          Responsible for planning and leadership interdisciplinary care planning weekly meetings.
-          Documents timely progress notes according to state and federal regulations.
-          Coordinates the discharge planning for residents.
-          Able to interpret and implement established social service policies, procedures and regulations.
-          Coordinates services of community agencies for continued delivery of care upon discharge.
