Full Job Description: Hiawatha HomeCare is a family-owned agency in Red Wing MN. We have been helping families stay at home together for over 30 years providing highly trained nurses up to 24 hours a day.
Reports to: Administrator
Primary Responsibilities: - Responsible for providing physical therapy to Home Care patients and case management when indicated. - Responsible for timely and accurate communication and documentation relative to physical therapy services. - Promotes and develops physical therapy services in homecare/hospice.
Specific Job Duties - includes, but not limited to: - Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment. - Perform initial start of care assessment and all related forms. - Develop, revise and, execute an appropriate care plan for restorative and maintenance patients. - Collaborate with physicians, case managers, nurses, home health aides, and office staff to ensure consistent levels of care. - Monitor patient progress and document accordingly. - Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment. · Knowledge of physical therapy principles, interpersonal dynamics, community resources, and legal limitations within the practice of physical therapy. · Attends on-the-job and in-service training. · Attends clinical and all other meetings accordingly. · All other duties and tasks as assigned.
Experience: · Demonstrated ability to multi-task.
· Ability to work and communicate professionally with patients, medical partners, employees, and supervisors.
· Ability to work independently and as part of a team.
Job Types: Full-time, Part-time, Contract
Salary: $42.00 - $60.00 per hour
