Required Skills:
Requires a Bachelor’s degree in Global Health, Public Health, or a related field. Must also include six months of related experience.

Experience must include:

• Experience with direct patient contact as a Community Health Worker.

• Experience documenting in electronic health records and following HIPAA and other security procedures.

• Experience working with communities experiencing health disparities or disparities in health care access.

• Experience working with diverse populations or refugees who recently resettled.

• Experience conducting home visits and community outreach to underserved populations.

Must have a Community Health Worker certificate from a Community Health Worker training program in Minnesota.

Must be willing to travel around the Twin Cities metro area. Travel is 20% of the time. Remote work is permissible.

Job Description:
Responsible for providing direct Community Health Worker (“CHW”) services to newly arrived immigrants who relocated to Minnesota for humanitarian reasons. This position will also train, mentor, and supervise a small team of trainees and coordinate data gathering and reporting protocols and processes for the Minnesota Department of Human Services – Resettlement Programs Office (“DHS-RPO”) team.

Specific duties include:

• Provide direct CHW services to patients experiencing a variety of health conditions, including health education, self-management skill building, and connections to community resources.

• Conduct patient visits at home and community sites, telephonically, and via telehealth platforms.

• Manage the work of a team of part time and full time CHWs.

• Assist company co-founders in refining existing practices and developing new models of patient care.

• Participate in regular check-ins with Medical Director and RN Health Coach to review patient progress and needs.

• Assist company co-founders with training, innovation, and building CHW team expertise.

• Work with co-founders, DHS-RPO staff and the CHW Solutions DHS-RPO team to develop and implement assessment questions, referral and documentation processes, and reporting protocols for services delivered to Minnesota Newcomers, including working within the Minnesota Refugee Information System (MRIS) and CHW Solutions’ Electronic Health Record.

• Train the CHW Solutions DHS RPO team on data requirements and coordinate data gathering and reporting practices.

• Schedule and facilitate regular check-ins with CHWs to provide guidance, constructive feedback, and tools.

• Work with company co-founders to manage projects and referral streams.
