Visits patients in counties - Jackson, Swain, Cherokee, Macon, Graham, and Clay - Mileage Reimbursement included
Position Summary:
Provide excellent, professional day-to-day hands-on personal care to individuals with chronic, serious, or life-limiting conditions. CNA's must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care.
Essential Duties & Responsibilities:
Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
Practice within the scope of license, certification or registration, where applicable, and as required by law and regulations as delegated to and directed by the registered nurse:
Participates in the coordination of the patient's hospice care in accordance with professional standards of practice
Collects data for ongoing interdisciplinary comprehensive assessments
Participates in the development and evaluation of the plan of care by reporting observations
Contributes to patient and family counseling and education
Perform duties in compliance with Medicare Condition of Participation
Practice safety and infection control methods
Provision of hands-on personal care

Must be willing and able to care for patients with infectious disease processes unless approved accommodations

Minimum Qualifications:
Education:
Experience:
- No more than a 24-month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course
- 0 - 1 year of clinical experience.
Licensures & Certifications: (if applicable)
- Valid NC CNA certificate
- Valid driver’s license and proof of automobile insurance
Summary of Physical Requirements:
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Flexion/Extension of arms/wrist
- Hand dexterity to include gripping, pushing/pulling and writing
- Lifting/carrying of 100 lbs. often
