About the Position:

The Registered Nurse Case Manager would be one of the EBI care team members of this intensive, adaptive, and client-focused care model. EBI is seeking a registered nursing professional that understands the importance of high-touch interactions between the client and healthcare provider to: enhance client healthcare access, health education, improve health outcomes, reduce utilization, and improve the overall well-being of the client.

As a Registered Nurse Case Manager, you would manage the medical needs of a caseload of patients throughout Alameda County as they navigate the staged progression of chronic kidney disease (CKD). Registered Nurse Care Managers would manage acute and post-acute episodic interventions, including condition assessment, SNF coordination, education, etc. in the patient’s home and virtually. Collaborate with the Care Team and physicians to prevent readmission, prevent future hospitalization, and improve patient care.

NOTE: SPANISH LANGUAGE PROFICIENCY IS REQUIRED FOR THIS ROLE, WITH THE ABILITY TO PASS A LANGUAGE ASSESSMENT.

Education / Experience:

  • Degree in Nursing is required.
  • Active Registered Nurse License - California Board of Registered Nursing (BRN).
  • Two years of RN experience minimum.
  • Experience working with chronically ill (CKD/ESRD) patients.
  • Home care, case management, and/or hospital experience preferred.
  • RN’s with acute care experience within the past 5 years is preferred (ICU, CCU, ED, Stepdown, Nephrology, Renal Transplant or Dialysis RN’s).
  • Spanish language fluency, and the ability to pass a language proficiency exam.

Locations:

  • This role will require up to 80% weekly travel throughout Alameda County. Mileage is reimbursed at the IRS rate of $0.70 per mile.
  • Possession of valid California driver’s license with a driving record that meets agency standards, auto insurance, and access to a vehicle.

Schedule:

  • Part-Time: 24 hours per week during business hours, Monday-Friday.
    • Schedule determined based on candidate's availability.

Compensation: 

  • $67.56 per hour.

Duties and Responsibilities:

  • Build strong, productive relationships and maintain consistent communication with patients, their families, and providers.
  • Assess and assist patients for linkage and utilization of appropriate medical and community resources, in conjunction with other members of the care coordination team.
  • Complete timely and accurate documentation per company procedures, relevant professional, and regulatory standards.
  • Collaborate with Care Team and physicians to prevent readmission, prevent future hospitalization, and improve overall patient care.
  • Provide introductions and high intensity engagement with patients and family members. Explain the Nurse Case Manager role, and process to contact Nurse Case Manager for questions, guidance, and education.
  • Foster a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions.
  • Manage a caseload of nephrology practice patients and perform clinical needs assessments, complex medical management, for high-risk CKD and ESRD patients.
  • Educate patients using kidney disease-specific education developed to be culturally appropriate, and tailored to a diverse set of learning needs.
  • Act as a patient’s advocate, liaison, and information resource; escalating all appropriate information to their nephrologist as appropriate.
