Job Description:
Direct Support Professional
Opportunity in a residential setting - all shifts available.
Summary:
Responsible for providing quality care to program participants by implementing the plan of care for the individual.
Essential Duties and Responsibilities:
- Operation of computer, phone and office equipment for business/service, light lifting associated with day-to-day operations, good communication skills and ability to provide transportation.
- Provides quality care to program participants in an environment that promotes their rights, freedom of choice and individuality.
- Must become knowledgeable about the Individual Service Plan (ISP) and provide care according to the participant’s plan.
- Responsible for maintaining the comfort and privacy of the participants in the delivery of care, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, transferring, ambulation and assistance with communicating their needs.
- Performing household responsibilities of the program, which may include assistance with grocery lists in conjunction with dietary needs, grocery shopping, meal planning, cooking and cleaning of the home.
- Completes necessary documentation according to the standards required by TWi.
- Assists with processing of Unusual and Major Unusual Incidents that may occur in program.
- Must be able to communicate effectively with participants, colleagues, family members, guardians, case workers and community members.
- Provides crisis assistance for individuals as may be necessary.
- Assists in training of individuals.
- Responsible for providing transportation for participants to appointments, errands and community events. This includes walks around the neighborhood for community interaction.
- May be responsible for development of the ISP in coordination with the SSA.
- Communicates with SSA staff for the purpose of coordinating county board programming. Ensures services are being provided in accordance with Individual Service Plan. Works in conjunction with Service and Support Administration staff to ensure communication with individuals, families and providers.
- Ensures compliance and quality assurance, when necessary, with all applicable federal, state and local rules, regulations, board policies and accreditation standards.
- Must review Individual Service Plans and accurately monitor hours of service provided. This may include data entry; type of service provided during the day/night.
- Ability to independently problem solve and offer solutions.
- Ability to remain patient and adapt to ever-changing situations that may occur.
- Must comply with all policies and procedures set forth by TWi.
- Must be able to work with a team, but also able to work independently.
- Responsible for being a role model by displaying appropriate decorum, dress, hygiene and attendance.
- Fully understands the rights of participants and promptly reports any evidence of abuse or any suspected violation of a participant’s rights.
- All other duties as assigned.
Qualifications:
High school diploma or GED. Experience working with adults with disabilities required. Must be willing to obtain the following certifications: Crisis Intervention, First Aid/CPR and Medication Aid within 60 days of hire.
Satisfactory criminal background and Abuser Registry checks and pre-employment drug test. Must possess and maintain a valid State of Ohio vehicle operator license.
All applicants must possess the required registration from the Ohio Department of DD, be eligible for the required registration having completed all the requirements as specified by DODD at the time of the interview or be willing to complete the requirements for registration as specified by the DODD within the required time frame. Bureau of Criminal Investigation check must result in no felony record. Drug screening must result in no substance abuse.
Language Skills:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Mathematical Skills:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference and volume.
Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills:
To perform this job successfully, an individual should have knowledge of internet software, spreadsheet software and word processing software.
