Required Technical Skills, Experience, Education, and Credentials:
- Bachelor's Degree or related experience is required in the field of Social Work, Counseling, or related Human Services Field
- Minimum of 1 years’ experience working with adults with severe and frequent mental illness
- Access to a personal vehicle and motor vehicle record acceptable to program and/or contractual requirements
- A valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage.
- Must be familiar with Microsoft Word and Excel.
- Effectively communicates complex ideas and instructions both verbally and in writing.
- Reliability in reporting to work regularly and on time.
- Professional workplace appearance and conduct; friendliness and courtesy to consumers, coworkers and vendors.
- Knowledge of CARES and Forward Health Portal is desirable.
- Basic understanding of Social Security benefits.
- Basic understanding of health insurance benefits.
Work Environment and Physical Requirements

- Work is performed largely in an office environment utilizing various office equipment including, but not limited to, a personal computer, copy machine, fax machine, and multi-line phone.
- Hours of work will generally be during regular business hours.
- Ability to communicate effectively over the telephone, by computer and in person
- Ability to perform well in a fast paced work environment.
