The Licensed Social Worker helps Transitonal Care Unit residents and family understand and cope with the psychosocial aspects of their lives involving change, grief, and loss through crisis intervention - individual, group and family counseling. Located in Edina, MN.

• Full Time Hours; 80 Hours Per Two Week Pay Period

• Coordinates admissions with the admissions team as appropriate
• Manages resident caseload
• Provides one to one and group counseling as appropriate
• Serves as a Discharge Planner
• Assists in Vulnerable Adult investigations and completes necessary documentation and reports.

Bachelor's Degree from an accredited school of social work
Licensed by the MN State Board of Social Work
One to three years experience in LTC
