Community Inclusion Specialist

POSITION SUMMARY:  This position assists residents with mental health disabilities to transition into the community via participation in a variety of community activities by enhancing the participant's life experiences and providing support through development and implementation of community integration projects.  This role also seeks out additional volunteer or vocational opportunities for residents within the community.

DUTIES AND RESPONSIBILITIES:
Assist supervisors with development, organization and facilitation of CI activities including but not limited to volunteer opportunities "Meals on Wheels", , in the local library, soup kitchens and the Society Prevention of Cruelty to Animals (SPCA) and leisure activities including and not limited to beach trips, movie outings, museum trips  to enhance their independence
Transport residents using Elwyn vehicles to and from community and recreational activities
Coordinate activity schedule/calendar with other CI team members to ensure the residents have opportunities for the residents to engage in volunteer and other opportunities
Train and supervise residents in the completion of work/volunteer activities by modeling specific behaviors (e.g. serving food, making sandwiches, filing books, cleaning computers)
Promote positive community growth through collaboration with volunteer and vocational partners, networking in the community, and utilizing community resources to assist residents' growth and independence
Communicate planned community activities to all ARBHS program staff through the distribution of the activity calendar (via email and by posting it at each program), using the house communication log and in staff meetings
Document and maintain all necessary data pertaining to community inclusion activities (e.g. date of event, name of activity and participation result) on the individual therapeutic activity log
Assist, along with the other CI and management staff, in the planning and coordination of all ARBHS annual resident events e.g. the summer and holiday galas, "Elwyn Idol" and the Mental Health Month (May) picnic
Assist in the development of wellness and recovery plans, based on the regulatory requirements of the ARBHS program, and other behavioral plans in coordination with the Clinical Director and program team, as needed
Assist with direct care duties as needed and requested by management staff
Perform cardiopulmonary resuscitation (CPR), safe crisis intervention (SCM) and medication administration using agency-trained protocols
Perform other duties as assigned

DIRECT REPORTS:  None

CONTACTS:  Elwyn staff, Elwyn residents, funding and regulatory agency personnel, families, other health care providers, area churches, community members/organizations, volunteer organizations, vocational contacts

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
High School diploma or equivalent required; associate degree or 60 credits in human service field preferred
Experience working with people with behavioral health disabilities, preferred
Community services experience, preferred
Demonstrated ability to adapt to changes in work load, assignments and schedule
Ability to flex schedule to meet resident needs
Demonstrated ability to work effectively as part of a team
Demonstrated strong time management and organizational skills
Must be able to perform CPR, SCM and medical administration using agency-trained protocols
Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
Demonstrated basic experience with Microsoft Office applications, including Word, Excel and Outlook
Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
