This position partners with patients and families to ensure they understand follow-up needs and coordinates their medical appointments, as well as supports the families in a holistic approach and be a point of contact with complex needs.
Rare Disease Program Manager Overview
Key responsibilities include but are not limited to:
Serves as a single point of contact for coordinating appointments, procedures, labs, imaging, or treatments
Accurately navigates and translates complex service protocols, tools, standard work, and information to ensure appropriate services are delivered
Gather and document information to help facilitate communication between the patient/caregiver and care team
Maintains awareness of multiple competing situations needing attention and prioritizes to assure patient needs are met, while maintaining a pleasant, friendly demeanor.
Identify potential issues that could complicate care (i.e. insurance issues, adherence concerns)
Review referrals to the program and determine if patient meets criteria to be a part of the Rare Disease program
Establish collaborative relationships with healthcare teams outside pediatric specialty and M Health Fairview
Minimum Qualifications:
Bachelor's degree in Business or Healthcare
5 years of related experience
Advanced experience with using Microsoft Excel, including pivot tables, formulas, and conditional formatting
Intermediate experience with Microsoft PowerPoint, including charts, animations
Location:M Health Fairview Pediatric Explorer Clinic - Minneapolis, MN
Hours:1.0 FTE
