Group Home Liaison! PSG/CIP is a community-based social services
organization looking for talented professionals like you to join our
team. We are currently seeking a dedicated individual to join our
Intensive Aftercare Program (IAP) team. IAP provides services to
families where children have been placed into out-of-home care. As the
Corrections / Group Home Liaison, you will work with youth who have been
placed into corrections, in the ACE (Alternative to Corrections through
Education) Program, or in group homes. The Liaison works with the youth
and the institution to ensure that an effective plan of care is
established to address the current treatment needs so that the youth can
be returned home or to a less restrictive placement as quickly as

RESPONSIBILITIES OF THE CORRECTIONS / GROUP HOME LIAISON: Analyze client
situations, capabilities and problems to determine what services are
needed. Assist with putting together and evaluating the treatment plan.
Develop aftercare plan for when the youth is returned home. Monitor
client progress in programming by conducting in-person visits with
clients a minimum of one time per month. Coordinate home visits between
youth and their families. Maintain monthly contact with client\'s
parents/guardians to provide progress updates. Coordinate and facilitate
a parent trip to each institution a minimum of one time per month.
Safely transport clients as needed. Exercise judgment to intervene as an
advocate for clients in emergency and crisis situations. LOCATION:
Position is housed out of our Sunnyside office in Kenosha, WI. Local
travel is required to meet clients and families at home and at the
corrections facility, group home, or other community spaces, with
mileage reimbursement. Must be able to meet in-person with families
located in Kenosha County, Wisconsin SCHEDULE: Monday-Friday
8:00am-5:00pm with occasional evenings to accommodate families
schedules. Rotating Saturdays are required about once or twice per
