FT: LPN Fredonia, NY Split shift, must be available for weekends. Oversee and promote the residents health and wellness through administration of all residents medications, regular visits with each resident, resident monitoring, and ongoing communication with the resident care team. The LPN uses his her nursing knowledge to insure the Resident needs are met DUTIES: Responsible for overall supervision of the building in the absence of management. Must be familiar with and able to implement emergency procedures as outlined in Emergency and Disaster Plan. Responds to requests for information and all other activities as directed Maintains on-going infection control surveillance, OSHA standards, universal precautions and basic sanitation & safety in the facility Assures accuracy of Resident records and documentation in adherence with company policies and regulations of the NY State Department of Health Assures the medication management system adheres to Federal, State, local Department of Health laws and regulations & company policies. Follows the policies and procedures for incident reporting and Department of Health Regulations. Maintain ongoing communication with the Director of Resident Care, Health & Wellness, Case Manager, resident, resident family and physician regarding resident medical needs. Resident Supervision: Acts as Resident advocate Provide medication assistance to residents Visits each Resident to assess concerns Investigates and resolves all Resident complaints Provides oversight in admission process Initial and ongoing orientation to facility for residents Acclimate and introduces residents to other residents. Encourages participation in social activities. Conducts or delegates as appropriate, quarterly, annual, and change in status assessments Follows and assists Resident Care Aides in following each Resident's Functional Assessment Communicates change in Condition to the Director of Resident Care, Director of Health Wellness Ensures all corporate policies, procedures and NY State Department of Health Regulations for Adult Homes and Assisted Living Residences (inclusive of Enhanced Services & Special Needs Programs) are followed Responsible for direct supervision of Resident Care Aides on Memory Care to ensure residents receive appropriate care, activity program, services, and nutrition with the Director of Resident Care and/or Director of Health Wellness; Coordinates care and medication with the Director of Resident Care, Director of Health Wellness, Case Manager, Resident's physician and 3rd party providers. Collaborates with the Director of Resident Care, Director of Health Wellness, Case Manager and to assist residents with referrals to pertinent agencies when needed; PT, OT, Speech, etc. Acts as a liaison with Resident's personal physician & other healthcare providers. Refers Resident service problems to the appropriate Department Managers as necessary. Develops and keeps accurate Resident records in compliance with company policy and regulatory rule: Maintains Nursing Notes to include necessary medical/nursing, mental health, needed interventions, and eventual outcomes. $23/hour QUALIFICATIONS: Graduate from an accredited school of nursing Licensed in State of New York as a Licensed Practical Nurse or Registered Professional Nurse (where required) Must speak/read in English Subject to pre employment drug testing, medical exam, reference and criminal background checks Apply in person: Fredonia Place, 50 Howard St., Fredonia, NY 14063
