JOB REQUIREMENTS: Duties Incumbent will work within the HUD-VASH program
as a Substance Use Disorder (SUD) lead. The incumbent will act as a
consultant for HUD-VASH social workers who serve Veterans with complex
substance use disorders. The incumbent is also responsible for providing
treatment for a caseload of veterans in the HUD-VASH program who have
high-risk substance use disorders. Responsible for following local
policy and SOP for the Medical Center and the HUD-VASH Program. Actively
participates in program planning and development, advising the
supervisor of shifting trends with caseloads and identifying the need
for and making recommendations on program procedure or policy changes
based upon work experiences. Takes responsibility for improving
professional skills, participating in planning and executing specific
staff development projects, attending seminars and institutes, and
participating in training programs of other disciplines as requested.
Works with VA Medical Center multi-disciplinary teams and community
agencies to strive for developing more effective and efficient ways of
delivering healthcare services to HUD-VASH Program veterans. Performs
work related to the review, monitoring, analysis, planning and
assessment of the HUD/VASH program. Independently performs case finding
functions and outreach, identifying and screening patients for
psychosocial needs. Performs clinical social work functions and
specialized clinical social work functions. Independently conducts
psychosocial assessments. Coordinates community-based services,
including information and referral for additional services from other VA
programs, other government programs and community agency programs.
Identifies high-risk patients and provides clinical care and services.
Provides crisis intervention services, seeking to address the cause as
well as the presenting complaint, coordinates family conferences and
serves as liaison to family members. Establishes and maintains effective
therapeutic relationships with veterans and their families. Provides
consultation services to other staff regarding the psychosocial needs
and substance use disorder treatment needs of veterans and the impact of
psychosocial problems and substance use on health care and adherence to
treatment plan. General knowledge of veterans\' benefits and services
relating to special programs, service connected compensation,
non-service connected pension. Incumbent meets the needs of customers
while supporting VA missions. Performs other related duties as assigned.
