Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. On this team, you will collaborate with physicians, other nurses, social workers, home health aides and volunteers to support patients wherever they call home. This position offers a flexible schedule and autonomy to work independently, allowing you to maintain a healthy work-life balance, 

A caregiver in this position works evenings from 11:00AM – 11:30PM. 

A caregiver who excels in this role will:  

  • Provide supervision, assessment, planning and direct patient care to hospice patients. 
  • Perform accurate and complete admissions assessments and re-assessments.  
  • Assess patients for safety risks, abuse and neglect.  
  • Assess, document and manage patient’s pain. 
  • Prioritize and document patient problems.  
  • Administer medications and treatments safely and accurately with a focus on patient comfort.  
  • Coordinate care for managing a caseload of patients.  
  • Document changes in patient condition and effectiveness and timeliness of care/interventions.  
  • Evaluate, update and revise plans of care.  
  • Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.  
  • Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs. 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of Nursing 
  • Current licensure as a Registered Nurse (RN) in the State of Ohio 
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).  
  • One year of current clinical experience as an RN 
  • One year of experience in an acute medical-surgical or critical care setting 
  • Valid Driver’s License (VDL) and proof of automobile insurance with $100,000/$300,000 coverage required 

Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN) 
  • Hospice and Palliative Care Nurse certification

Physical Requirements:

  • Full range of body motion for handling and lifting packages, manual and finger dexterity and eye-hand coordination
  • Standing and walking for extensive periods of time
  • Heavy work, exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently and/or 10 to 20 pounds of force constantly to move objects
  • Physical Demand requirements in excess of those for Medium Work
