Job Summary
Under general supervision, uses a client-centered or motivational approach
to behavior change and proven intervention models to assist clients recently
diagnosed with a chronic illness or clients with prior diagnosis and currently
out of care. Provides basic care coordination, identifies and reduces
barriers to care, and provides referrals to necessary resources based upon
individualized client needs and circumstances. Actively interacts with
clients in community, clinic, or correctional care settings.
