The Case Manager will work for our Seniors Division, and will provide case management services to seniors ages 60 years old and over. 

This is a great opportunity for a recent college graduate!

Responsibilities

• Provide an initial assessment of the senior adult's needs and develop a care plan for services and assist with the implementation of this plan.
• Provide monitoring and reassessment of all open EISEP case management cases to assure that all services are being delivered as planned
• Prepare reassessment forms for all EISEP clients receiving home care services
• Assist with the EISEP Case Management Program office coverage biweekly and as needed. 
• Provide information and referral to clients as requested.
• Provide advocacy for clients regarding entitlements and other basic needs
  Outreach within the community to provide information and assistance.

Qualifications

• Bachelor's Degree required
• Minimum of 1 year of case management experience
• Experience with geriatric population preferred
• The ability to do home visitation
• Excellent verbal and written communication skills
• Proficient computer skills with MS Office
• Valid and clean driver's license

Additional Information

Schedule:

• Full-time, 35 hours per week, Monday-Friday
• Salary: $26-28/Hour
• Location: Garden City
