The Program Manager for Targeted Case Management will be responsible for supervising a dynamic team of case managers, providing services for individuals receiving Rule 79 Adult Mental Health Targeted Case Management. The manager will be responsible for hiring and supervising mental health case managers who will provide all components of case management inclusive of general case management and care coordination through plan development, monitoring, and evaluation. Principle areas of responsibility include client engagement, goal development, referrals to resources, problem-solving and general life skills assessment, and development. The manager may be called upon to assume a direct care role as needed. This position may require the ability to work outside regular business hours on an occasional basis to meet client and program needs.
PRIMARY RESPONSIBILITIES:
Oversee program management for all Targeted Case Management and Housing First Case Management, to ensure high quality, mission-driven services.
Attend Leadership Circle, service area team meetings, and other key meetings
Address urgent client situations
In collaboration with Program Director, develop and maintain all programs, ensuring best practices are being used.
Develop formal program policies and procedures in collaboration team and approval by division leadership.
Provide regular individual and group supervision and team meetings along with clinical consultation, supervision, and education.
Actively participate in the division's Trauma Informed Care Initiative and motivational interviewing. Support and implement evidence-based practices related to specific program area.
Coordinate program staff efforts to achieve annual goals and instill a sense of accountability among team members
Ensure all programs and services provided for the benefit of the participants in the program are aligned with program principles and goals for the organization
Assist in other division-wide service areas, including on call coverage as needed
Monitor, measure, and report on operational issues, opportunities, plans, and achievements to Program Director
Develop and maintain collaborative working relationships with community partners.
Build, foster and ensure intra-divisional collaboration to maximize participant access to the organization's programs and services.
Communicate with team, supervisor, service area director and vice president about potential changes likely to impact services or program operations.
Actively contribute to problem solving to improve service delivery.
Maintain positive and timely communication with program funders.
Qualifications



POSITION REQUIREMENTS
Education and Experience
Required:
Bachelor's degree in one of the behavioral sciences or a related field from an accredited college or university.
3 years of supervision experience needed.
Minimum three years of experience as a case manager (including a minimum of at least 2,000 hours of supervised experience in the delivery of services to adults with mental illness).
Experience with and/or commitment to working in a diverse, continuum of services in a fast-paced, growing organization.
PREFERRED
Experience with programs serving individuals with dual diagnoses who have a history of long-term homelessness.
Licensed or dually licensed professional (mental health and chemical health).
Experience working in Harm Reduction and Housing First with a demonstrated working knowledge and commitment to these modalities.
Lived experience of homelessness and/or substance use disorder recovery.
