Description

For a youth and family therapist who comes at difficult problems in new ways, Nexus-FACTS has a unique opportunity to be part of a new, one-of-a-kind treatment team!
Nexus-FACTS seeks a hybrid remote Mental Health Therapist or Clinical Trainee for our new, grant funded Youth Care Transition (YCT) program. The YCT is a new opportunity to provide intensive support to youth and their caregivers as they transition out of a Psychiatric Residential Treatment Facility (PRTF), Child and Adolescent Behavioral Health hospital (CABHH), or an emergency room hospital setting. YCT program provides placement coordination services and intensive therapeutic services with the youth and their caregivers through a multi-disciplinary teamed approach. The Therapist provides high intensity, short-term in-home and community based services with a strong system's focus to develop and support family and community-based placements and connections. This position is a hybrid-remote model with ability to work from home outside of scheduled therapy sessions with youth and their caregivers.
Primary responsibilities:
Creates and drives a trauma-informed environment
Collaboration with youth and family to work with a team and develop a Nexus Individual and Family Guidance Plan
Engage in and have active involvement of the Family Collaborative Intake meetings
Complete Child and Adolescent Needs and Strengths (CANS) functional assessment in accordance with the TCOM (collaborative) principals
Provide brief, individualized, youth-focused, strength and evidenced-based family and individual therapy models with youth and families.
Participate in all youth related treatment team meetings
Assess readiness for discharge in conjunction with the treatment team
Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition
Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process
Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional manner according to policy and regulated statutes
Required Education and Licensure:
Master's degree in social work, psychology, or clinically related field
Licensure preferred or on track with licensure within related clinical field (LICSW, LPCC or LMFT) per state board requirements
Valid driver's license required
Nice to have:
Experience working with at-risk youth and families facing mental health challenges
Location:
Hybrid remote (community and home based) in the Twin Cities, MN/metro area!
Travel:
Regular business travel to family homes, community locations, and referral sites required
Schedule/Pay:
Monday-Friday schedule, no weekends at this time!
Starting at $60k annually for clinical trainee/unlicensed therapist level, $65k annually for fully licensed therapist
Benefits:
