Spectrum Healthcare Resources has a potential opportunity for a Registered Nurse-Case Manager at Wright Patterson AFB in Dayton, OH.

Minimum Requirements:

Degree/Education: Graduated from an associate or baccalaureate degree program in nursing accredited by the Accreditation Commision for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). HCWs are required to maintain Continuing Education as required by their State Board of Nursing.
Certifications: Must possess one of the following certifications; Commission for Case Manager Certification, Certified Case Manaer (CCM), Certification of Disability Management Specialists Commission, Certified Disability Management Specialist (CDMS), Association of Rehabilitation Nurses, Certified Rehabilitation Registered Nurse (CRRN), American Board for Occupational Health Nurses, Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S), National Board for Certification in Continuity of Care, Advanced Certification in Continuity of Care (ACCC), Commission on Rehabilitation Coundselor Certification, Certified Rehabilitation Counselor (CRC), American Nurses Credentialing Center, Nurse Case Manager (RNNCM), National Academy of Certified Care Managers, Care Manager Certified(CMC).
BLS though American Heart Association
Experience: 4 years of clinical nursing experience is required with 2 years of previous experience in case management or discharge planning. Full time employment in a nursing field within the last 36 months is mandatory. Experience in performing case management services for Mental Health diagnoses/patients is beneficial.

Duties:

Engage in all facets of the Case Management Program (CMP), ensuring adherence to established case management (CM) standards, including assessment, planning, implementation, coordination, monitoring, and evaluation.
Offer expertise in social work regarding CM processes, identifying opportunities, integrating local CM processes, and aiding in the development of tools for case management support.
Merge CM with utilization management (UM) for seamless coordination.
Facilitate referrals and maintain communication with community agencies and organizations.
Accurately document patient care data and develop comprehensive treatment plans.
Establish mechanisms to ensure treatment plans are implemented effectively and followed up post-discharge.
Provide health care instructions to patients and caregivers based on identified learning needs.
Devise strategies leveraging nursing and social work strengths for effective case management.
Assist in educational program development and implementation.
Contribute to interdisciplinary team meetings, facility meetings, and care coordination meetings, sharing relevant clinical and educational experiences.
Participate in staff orientation, training, and committee activities.
Maintain productivity and quality standards in line with facility policies, professional ethics, and relevant guidelines.
Adhere to applicable instructions, policies, and guidelines from DHA/AF/local MTF.
Complete required medical record documentation, coding, and data reporting.
Undergo mandatory electronic medical record and case management training.
Conduct nursing peer reviews as assigned.
Demonstrate self-sufficiency in identifying, planning, and executing case workload and projects, developing detailed procedures and guidelines as necessary based on analysis and review.
