Physical Therapist! This is your opportunity to provide care to patients
in Tri County\'s brand-new hospital. We serve patients in three primary
care clinics, a 24-bed critical-access hospital and in an. What you will
do: Provide a mix of Outpatient and Inpatient care Monday-Friday day
shifts, every 5th or 6th weekend (Saturdays only) Work full-time (80

is who we are. Job Description: The Physical Therapist in their clinical
role examines, evaluates and assesses individuals of all ages who have
medical problems or other health related conditions that limit their
ability to move or function with activities of daily living. The
Physical Therapist develops a plan of care and provides treatment
techniques and interventions to improve mobility, reduce pain, restore
function and prevent disability. The Physical Therapist provides
physical therapy services and elements of patient/client care management
leading to optimal outcomes according to the relevant state Physical
Therapy Practice Standards. The Physical Therapist works collaboratively
with a multidisciplinary team in a team in the areas of education and
consultation regarding patient needs in accordance with the vision and
mission of Gundersen Health System. Major Responsibilities: 1. Works
with patients and families to examine, evaluate, diagnose, prognose, and
intervene to insure optimal outcomes. Also consults and educates the
patient and other care-givers as needed. 2. The physical therapist works
with the referring provider and other clinical staff in providing
optimal care of the patient through appropriate timely methods of
communication including, but not limited to, complete and accurate
patient chart notes and face-to-face consultations. 3. Interacts and
practices in collaboration with a variety of professions, communicates
