Job Description

Registered Nurse Case Manager Hospice

Summary/Objective
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Follows applicable laws, regulations and established policies and procedures.

Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process. Uses health assessment data to determine nursing diagnosis.
Assesses and evaluates patient’s status by: Writing and initiating plan of care; Regularly re-evaluating patient and family/caregiver needs; Revises the plan of care as necessary.
Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Provides health care instructions to the patient as appropriate per assessment and plan. Counsels the patient and family in meeting nursing and related needs.

Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
Participates in on-call duties as defined by the on-call policy. Fulfills the obligation of requested and/or accepted case assignments.
Ensures that arrangements for equipment and other necessary items and services
are available.
Supervises ancillary personnel and delegates responsibilities when required.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Maintains the comfort, privacy and dignity of patients and interacts with them in a manner that displays warmth, respect and promotes a caring environment.
Communicates and interacts effectively and tactfully with patients, visitors, families, peers, and supervisors.
Attend and participate in departmental meetings and in-services as directed.

Supervisory Responsibility
This position may have direct supervisor responsibilities for Certified Nurse Aides and other personnel assigned.
Work Environment
May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions.
This role may be exposed to infections and come into contact with patients who may have contagious illnesses and under wide variety of circumstances. May be subject to varying and unpredictable situations. May handle emergency or crisis situations.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping on a regular basis. The employee must frequently lift and/or move objects and patients weighing over 75 pounds (ability to adequately lift, support, and bodily manipulate the weight of a patient). Fine hand coordination, ability to read and write, the ability to understand and follow written, oral instructions and directions. Ability to remain calm under pressure.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
Position Type and Expected Hours of Work
Rotating shifts as well as holiday and work during inclement weather will be required.
Travel
Travel is expected for this position.
Required Education and Experience

Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
Current State License as a Registered Nurse
C.P.R. Certified
Bachelor’s degree, with one (1) year of hospice care experience preferred.
Must be a licensed driver with an automobile that is insured in accordance with state/or company requirements and is in good working order.

Additional Eligibility Qualifications

Proficient computer skills.
Ability to read and follow written instructions and document care given.
Understands hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.

Ability to prioritize and manage multiple tasks.
Strong written and oral communication skills.
Must be able to maintain confidentiality regarding patient and company propriety information.
Must have the ability to relate professionally, positively and work cooperatively with patients, families, community, and other employees as all levels.
