Community Consultant/DSP:
The Community Consultant/DSP assists persons with disabilities in pursuing their personal futures and goals through residential supports, volunteer opportunities, and community interaction. DSP's/CC's support the emotional, physical and personal well-being of individuals with disabilities at their homes or their neighborhoods and communities. Their role is to enhance the ability of consumers to lead self-determining lives by providing the support and information necessary to build self-esteem, assertiveness and to make decisions. This is done through the use of effective communication strategies and skills to establish collaborative relationships with consumers. DSP's/CC's assist with planning, coordinating supports, providing direct supports, and developing and maintaining relationships. Monitor support plans. Provide flexible assistance as the needs of the individual changes. Assist and support people with disabilities in gaining citizenship, involvement in their communities, and the development of friends and services outside of the human service system.
Job requirements for this position are:
- High school diploma or demonstrated equivalent competency required.
- 18 Years of age or older
- Valid driver's license and dependable transportation.
- Completion of required training/education/certification
- Experience working with persons with disabilities in the community preferred.
- Excellent people and communication skills required.
- Ability to work flexible/non-standard hours.
- Ability to lift up to 35 pounds; upper body leverage strength required.
- Ability to keep accurate documentation.
- Knowledge of community and community activities preferred.
Team Lead:
As a Team Lead, you will assist persons with disabilities in pursuing their personal futures and goals through residential supports and community integration. You will assist with the design, implementation and monitoring of support plans to ensure meaningfulness. Team Lead'soverseeand coordinatethe day-to-day activities within assigned residential locations. They also review documentation to ensure progress is being accomplished toward ISP goals for individuals assigned. You will act as a mentor and provide support to thosestaff working in their assigned locationsby providing training and development to ensure successful outcomes for people served. Team Lead's are required to maintain case records and documentation for persons/locations assigned, including consumer-specific financial and medical records.
Job requirements for this position are:
- High School Diploma or GED required.
- At least one year experience working with persons with disabilities required.
- Previous supervisory experience preferred.
- Excellent verbal and written communication skills.
- Basic computer skills for data entry and e-mail.
- Demonstrated ability to organize a household and teach others.
- Ability to work flexible, non-standard hours.
- Valid driver's license and dependable transportation.
- Ability to lift up to 35 pounds; upper body leverage strength required.
Residential Field Consultant:
Assist persons with disabilities in pursuing their personal futures and goals through residential supports and community integration. Assist with the design, implementation and monitoring of support plans to ensure meaningfulness. Act as a mentor and support to ensure successful outcomes for people served through programs. Provide training, development, and support to all staff to ensure quality of services. Assist with overseeing medication administration. Maintain case records and documentation for persons/locations assigned. Provide safe and timely transport of individuals by operating a motor vehicle in compliance with all applicable DOT, Federal, State, local and system regulations. Uphold and promote the agency's mission statement. Carry out On-call duties as assigned.
Job requirements for this position are:
- High School Diploma or GED required.
- Be 21 years of age or older.
- Possess a valid driver's license, good driving record that meets or exceeds system's established driver eligibility standards and ability to obtain a public passenger chauffeur's license required.
- Ability to lift a minimum of 70 pounds, exert up to 20-50 pounds of force occasionally and/or 10-26 pounds of force frequently. Upper body leverage strength required.
