• Must have three (3) years of human services or other related work in place of a bachelor's degree
• Participant recruitment and outreach experience are preferred
• Bilingual in Hmong/English is required

Job Description:
Primary Function of the Position
The SNAP Outreach Coordinator is responsible for providing outreach and education services and increasing access to the Supplemental Nutrition Assistance Program (SNAP) to the immigrant and refugee communities. This role will be to plan and execute outreach activities in a variety of settings such as HAP’s internal programs, schools, churches, and community agencies. Primary responsibilities of this position include educating immigrant community members of all ages about the purpose, use and benefits of SNAP. Additionally, provide increased access to SNAP information with a focus on eligibility screening and providing direct SNAP application assistance. In addition, this position will assist in HAP’s Workforce Development initiative of SNAP & Employment training directed.

Position Responsibilities
• Develop and disseminate SNAP information primarily to Hmong, Southeast Asians and other refugee and immigrant populations reaching at least 3,000 people annually
• Conduct home visits as needed to provide education and application assistance.
• Identify and coordinate outreach events in the communities served by HAP working collaboratively with other internal teams and with external partners.
• Work with Manager to develop outreach communication for PowerPoint presentations, outreach events and social media.
• Work with key staff to conduct internal SNAP screenings and referral system
• Screen and perform application assistance to qualified individuals and provide follow up assistance and on application status to clients
• Track and report outreach activities and outcome deliverables to supervisor in a timely manner
• Track and capture case notations daily into Salesforce
• Manage an average caseload of 25-35 clients
• Other duties as assigned

• While performing the duty of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer, phone, and keyboard
• Must be able to remain in a stationary position for sustained periods of time in an indoor setting
• Ability to tolerate moderate noise levels (i.e., shared open office space with computers, phone, and printers, light traffic)
• Must have the ability to move about the office to access files and office equipment
• Must have access to reliable transportation for occasional travel for meetings at other HAP office locations, events, and offsite meetings
• Occasional moving or lifting of boxes or files up to 20 pounds
• Frequent interaction with coworkers and participants requiring the ability to communicate effectively
• Frequent communication via email requiring the ability to read and write in English fluently
