Responsibilities

Ascension Health Services is looking for a full-time Nurse Practitioner for our Perry Hall Health Center location.

Responsibilities:

In collaboration with a physician, provide for the expansion of individualized primary healthcare service by diagnosing and treating certain acute or chronic health conditions including pediatric and adult populations.

• Obtains patient history and performs assessment via observation, interview and examination. Orders,

performs and interprets diagnostic studies. Performs preventative health assessments, screening,

immunizations and patient care. Performs annual wellness visits, annual physicals, well child visits, and pre-school physicals.

• Provides direct treatment and management of health conditions via prescriptive authority or referral to

other healthcare providers or community resources. Provides screenings for mental health diseases in pediatric and adult populations.

• Counsels and educates patients and their families/caregivers concerning preventative health, treatment

options and community resources.

• Documents delivery of health care and nursing processes in accordance with specific specialty area

standards and network nursing policy.

• Provides necessary SCOH screening and refers to appropriate social services when needed.

Requirements

Licensure/Certification/Registration:

  • BLS Provider required.

  • Nurse Practitioner credentialed from the District of Columbia Board of Nursing obtained prior to hire date or job transfer date required.

  • Drug Enforcement Admin preferred.

Education:

  • Master's degree of Nursing required.
