Responsibilities



At PrairieCare, our LGSW Social Worker team is responsible to assess, plan, implement, and evaluate the psychosocial services for patients of PrairieCare. The employee provides a comprehensive range of services to patients and their families, including assessment, treatment planning, care coordination/case management, and seeks to enhance the functioning of patients. This position may work with children, adolescents, and adults and serving patients with co-occurring disorders.
Hours for this role are Monday - Friday, 8a-430p
Compensation for this role starts at $27.75 and up based on relevant experience
Responsibilities and Accountabilities:
Accountable to have knowledge and understanding of Social Work practices and principles
Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groups
Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
Responsible for communicating with patients and families to keep them involved in treatment plan decisions
Coordinates care with outpatient providers upon admission and discharge
Responsible for facilitating weekly treatment team meetings and creating master treatment plans/updating treatment plans as agreed upon by the treatment team
Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
Communicate clearly and effectively with clinical team members and Directors
Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department






Qualifications



Master's Degree in Social Work with licensure in the State of Minnesota (LGSW) required.
Experience in healthcare setting preferred
Experience using electronic health records preferred
Knowledge of Microsoft Office Products including Word and Outlook
Able to effectively and efficiently communicate in written and verbal form
Ability to smoothly and rapidly shift between dissimilar tasks
Ability to communicate well both verbally and in writing
Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
Ability to make effective decisions while in stressful situations and circumstances
After training, be able to manage disruptive and explosive patients.
