Order#: NY1496244 Reentry Program Coordinator Summary: This position is responsible for ensuring individuals who are returning from incarceration or shelter have the support they need to be successful in the communtiy. Safe and adequate housing is a fundamental basic need, and a critical factor in reducing recidivism and/or return to shelter. Our Housing Specialists work hard to connect individuals not only to our 40-unit, scattered site project but also the wraparound supports they need to thrive. Key Job Responsibilities: Support individuals who are overcoming the challenges of returning to the community after experiencing homelessness or a period of prolonged incarceration Assist program participants in locating safe and sustainable housing Provide direct case management for those participants ineligible for case management through other community organizations Carry a caseload of approximately 20-25 clients Develop and maintain strong relationships with local landlords to ensure ongoing and future availability of housing Maintain contact with participants on caseload on a regular consistent basis, and a minimum of one time per month Conduct office, home and community visits in partnership with other team members Conduct and document thorough, annual assessments for program participants Work with individuals to identify other needs, and coordinate services at Delphi Rise or with other providers to ensure warm referrals Participate and actively engage in monthly Delphi Care Services Case Review Maintain accurate and timely records in accordance with program requirements utilizing the required data platform Perform other tasks as assigned Qualifications: Associates degree in a field relates to human services or criminal justice is strongly preferred; equivalent work experience will be considered Up to two years of experience working with clients with criminal justice involvement, mental illness, homelessness and/or addiction is required Familiarity with and understanding of appropriate resources in the community is needed Strong communication and decision making skills are critical Must have reliable transportation and a clean license Candidate must demonstrate a commitment to working as part of a diverse team and to provide culturally appropriate services to a diverse client population
