Description

This Clinical Pharmacist role will be focused on Med Histories at Cleveland Medical Center.

Position Summary

The Clinical Pharmacist is responsible for the safe, effective, and cost-effective use of medications through the application of specialized knowledge, skills, and professional judgment. The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patient's disease conditions, background, need for understanding and participation in drug therapy in a fiscally responsible manner. These activities occur in all aspects of pharmacy practice including adult and pediatric inpatient areas, operating room pharmacies, ambulatory oncology and outpatient services (ie., Bolwell Pharmacy). The pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific pharmaceuticals pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and/or 4741.

Essential Functions


Documentation and performance of medication reconciliation
Inventory control and adherence to Hospital Formulary policies
Adherence to Hospital and Departmental Policies and Procedures
Responsible for obtaining comprehensive preadmission medication history from multiple available courses and patient interviews.
Documenting the compiled medication list, thoroughly analyzing possible drug interactions, evaluating patient compliance, and any medication adherence barriers.
Summarizing all gathered info in clinical notes and being able to express yourself clearly in professional writing
Being a productive and efficient team member to meet daily goals and expectations
Overseeing PGY1 pharmacy residents' daily medication reconciliation process and serving as a clinical preceptor and mentor. To effectively communicate with the residents, provide constructive feedback, and process improvement suggestions.
