Job Description:
This position will provide leadership for the Care Management staff, including case managers, social workers, support and other affiliated staff. To assure the successful development and implementation of care management strategies as they relate to the care management role. To act as a clinical and process consultant for care management staff regarding care coordination, concurrent review and transition/discharge planning. Actively support hospital care and system improvement initiatives with increased focus on identified areas of opportunity with care management and affiliated activities.
Work Schedule: 1.0 FTE
Required Qualifications:
Master's Degree in Social Work OR Master's Degree Nursing OR currently enrolled in a Master's Degree program with an established date of completion required.
Minimum of 3 years clinical practice experience in health care setting.
Minimum of 3 years in utilization review and discharge planning, transitions with complex populations, or quality improvement.
Minimum 2 years leadership/management experience.
Dual licensure from Minnesota and Wisconsin Boards of Social Work OR
Dual Nursing licensure from Minnesota and Wisconsin.
Must be licensed in both states within six months of hire date.
Demonstrated effective leadership and supervisory skills.
Understanding of healthcare and/or managed care industry.
Demonstrated experience in hiring, orientation, coaching, and team building of professional staff.
Demonstrated flexibility and organization.
Demonstrated appropriate decision making under challenging situations.
High level of expertise in written, oral, and interpersonal communication.
Demonstrated skill in effective use and management of automated medical management systems.
Preferred Qualifications:
ACN, CCM specialty certifications
