Our planning and service coverage area includes Appanoose, Clinton, Davis, Des Moines, Henry, Jefferson, Keokuk, Lee, Louisa, Lucas, Mahaska, Monroe, Muscatine, Scott, Van Buren, Wapello and Wayne counties in Iowa.

Milestones Area Agency on Aging is committed to working with communities and community-based organizations to increase awareness for aging successfully with dignity and independence.  We do this by implementing policy with public resources provided by the Federal Administration on Aging and the Iowa Department on Aging.

Services We Offer...

Information, Referral, and Assistance provides a connection to community networks and resources
to help people age 60+, and individuals living with disabilities age 18-59 maintain independence, security,
and quality of life.

Care Coordination/Options Counseling provides short-term individual planning and implementation
of long-term support and services for people 60+, individuals with disabilities 18+, veterans, and caregivers.
Case Management is the ongoing coordination of services for those 60+ living independently at home by
utilizing resources for home-based community services that help with activities of daily living.

Senior Nutrition Program offers daily nutritious meals to individuals age 60+ and their spouses of any
age. Meals are served in social, congregate dining centers known as “Community Cafés,” participating “Iowa
Café” restaurants, or delivered to the home for those who are home-bound. Individual nutrition counseling and group nutrition/wellness education is also offered.

Family Caregiver Program supports those who provide care for anyone age 60+ by providing information and resources along with individual counseling and support groups.

Iowa Return to Community Program helps facilitate a successful transition back to home and community after a hospital or rehabilitation stay.

Senior Health Insurance Program Information connects experienced, knowledgeable volunteers with individuals to help provide guidance and possible savings for Medicare and other health insurance programs.

Wellness and Safety: Heath Promotion and Evidence Based Programs are facilitated by trained leaders to help reduce falls risks, manage chronic health conditions, and improve health outcomes. Format is typically small group, multi-week class series, held in-person or online. Current programming includes such courses as as “Tai Chi for Arthritis and Falls Prevention” and “A Matter of Balance.”

Elder Abuse Prevention and Awareness Resources, information, and proactive solutions are made
available for individuals and families that have concern for their safety and well-being. Training is also
available to businesses to help employees recognize potential physical, mental, or financial exploitation and
tips on how to respond.

Older Worker Employment Program provides an array of employment and supportive services to
those age 55 and older with a disability who have an open case with Iowa Vocational Rehabilitation.
