What skills do I need to bring?

Must be licensed as a Registered Nurse (RN) in Minnesota
Basic Life Support (BLS) CPR course for healthcare providers through American Heart Association or Red Cross renewed at recommended frequencies

Must be fluent in the English language
Valid U.S. driver’s license with minimal moving violations and access to a reliable personal vehicle

Position Summary

Are you a Registered Nurse (RN) looking to provide 1:1 care, manage your own schedule, and grow your skills? Accurate Home Care, and its Home Health service line, is growing and adding an RN Case Manager to its team.

$76,000 - $87,000 per year

What will I be doing?

Complete patient visits as needed under the direction of the Director of Home Health.
Participates in the implementation and oversight of nursing and home health aide services.
Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge.
