is HYBRID. FTE: 0.900000 Shift: Shift 1 Home Health nurses provide
skilled care to patients in their homes or home environment. A
successful Home Health nurse will demonstrate flexibility and patience
in order to provide care in a variety of living environments. Strong
organizational skills along with solid clinical experience are necessary
for successful care delivery. One on one focused patient care is the
core of Home Health nursing and one of the most rewarding components of

Summary: Plans, organizes, coordinates, and provides direct nursing care
for an identified group of patients according to specific standards. JOB
REQUIREMENTS/PREFERRENCES: Graduate of an accredited school of nursing
required, and current unrestricted Wisconsin RN license required. One
year experience in home care or hospice or equivalent case management
experience. Excellent customer service, communication, organizational,
problem-solving and time-management skills required. Must maintain
current driver\'s license and personal auto liability insurance. CPR
