Description

Providence Place, a Lifespark Skilled Nursing Community, is looking for a dedicated and passionate Clinical Support Specialist (CSS) to join our growing team.

Providence Place is conveniently located on the bus route, located in southern Minneapolis in the Standish Ericcson neighborhood. Here we provide services for our seniors in a TCU (Transitional Care Unit) Care, Long-Term Care and Memory Care. We have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting senior.
The position is responsible and accountable for mentoring/teaching nursing staff the knowledge and skill necessary to administer exemplary care, providing supervision, consultation, and assistance to nursing staff/other disciplines in the assessment, planning, implementation, and evaluation of care. Promotes excellence in customer service.
In this position, you will be providing support to our residents with their personal care services including:
Skills in planning, organizing, directing, communicating, motivating, and problem solving.
Knowledge of State and Federal regulations as they apply to long term care.
Has good leadership skills.
Visits, monitors, and follows up on all new admissions.
Investigates incidents and complaints and takes appropriate action in consultation with the Assistant Director of Nursing.
Ensure compliance with established infection control practices.
Prepares monthly reports such as but not limited to incidents, audits, and infections in a timely manner.
Checks resident "plan of care" for updates, significant changes, and short-term problems.
Performs "rounds" one to two times daily to include Unit Care Manager and staff.
Participates in unit "stand-up" meetings daily with AM and PM shifts.
Qualifications
AA or BA/BS- in health care, social service, business
Minimum of 3 years related experience in health care education, program development, or healthcare leadership.
Experience in a health care setting is preferred.
Must possess basic computer skills, including email use, word processing, basic spreadsheets.
