Position Overview:

We are seeking professional, self-motivated Clinical Assistants (LPN or CMA) to join our team that delivers primary care services to members within the comfort of their own home.  The Clinical Assistant provides collaborative nursing support working alongside a provider team traveling to group homes in the Southeast Metro and Surrounding Areas near Eagan, MN (Prior Lake, Apple Valley, Bloomington, Edina, Richfield, Mendota Heights, Inver Grove Heights, Cottage Grove and St. Paul, MN).

Duties include facilitating an efficient, organized clinical flow, pre-rounding, chart prepping, medication reconciliation, keeping EMR up to date and assisting with provider paperwork. 

Full time hybrid position, day shift hours, no evenings, weekends or holidays.  Hours are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays. Role is typically in the field patient facing 2-3 days per week for 2-3 hours and the remining time is remote (work from home).

Responsibilities:

  • Provide patient care support, assist providers as needed and facilitate an  efficient, organized clinical flow
  • Interact with patients for annual wellness visits and document as necessary
  • Pre-rounding and/or rounding with providers while updating EMR
  • Ensure electronic medical records are accurately updated on a daily basis
  • Set up e-prescriptions under direct supervision of providers
  • Medication reconciliation
  • Work independently and with provider teams, internal staff, and facility staff
  • Take notes and document observations alongside provider during patient visits
  • Monitor patient portal messages, summarize for provider as necessary

Qualifications:

Education/Certification/Experience

  • Graduate of an accredited Medical Assistant Program or Licensed Practical Nurse Program
  • Active Licensed Practical Nurse (LPN) license required or Medical Assistant certification (CMA) or registration through accredited program required
  • 1-2 years experience having worked in a clinical setting preferred

Knowledge/Skills/Abilities

  • Medical terminology knowledge and understanding of patient care notes
  • Technology and Computer proficient, electronic medical record systems experience preferred
  • Excellent interpersonal and customer service skills
  • Detail oriented and accurate
  • 45+ WPM typing,
  • Demonstrated compatibility with Bluestone’s mission and operating philosophies
  • Demonstrated ability to read, write, speak, and understand the English language
