About Us:
Avivo's Ending Homelessness division provides pathways to housing for individuals experiencing homelessness. We work with county coordinated entry systems and tribal governments, as well as operating unique street outreach teams to connect unsheltered individuals with shelter and housing. Avivo's Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma-informed service delivery.
What You Will Do:
By joining our team as a Case Manager, you will make a direct impact on the lives of diverse individuals in our site-based housing programs, American House, and Kimball Court. In this role, you'll work within our Harm Reduction and Housing First programs, providing personalized support to residents to help them achieve housing stability and their personal goals. You'll be at the forefront of a social justice issue, ensuring that safe housing is recognized as a fundamental human right.
We're looking for someone who thrives in dynamic environments, excels in meeting people where they are, and is dedicated to delivering high-quality, culturally responsive, and evidence-based services.


Engage and build rapport with clients.
Assess eligibility for program and services and connect clients.
Develop Goal Plan.
Make appropriate referrals, monitor progress, and ensure follow-up.
Utilize creative approaches to meet in person, regularly with clients and address their needs.
Immediately consult about any safety related concerns (for client and staff), mandated reporting concerns, and complaints from the client.
Provide housing specific services and build landlord relations.
Paperwork/Documentation/Data/Compliance completed on time.
Develop Crisis Plan and conduct crisis intervention as needed.
Ensure services provided are culturally responsive utilizing best practices.
Qualifications




What You Will Bring:

High School Diploma or GED required.
One or more years of experience working with single adults experiencing homelessness or similar experience.
Lived experience will be valued and considered as part of the experience requirement.
Must have a valid, current driver's license, access to a personal vehicle, proof of valid insurance and a clean driving record. Mileage reimbursement will be provided.
Able to pass DHS background check.

Preferred Qualifications:

Openness to Harm Reduction and Housing First and a willingness to learn how to use it.
Basic knowledge and awareness of mental health, chemical health, community resources, trauma, institutional racism, gender issues, tenant's rights.
Knowledge of community resources or where to find them.
Ability to proactively advocate.
Ability to work with individuals different than themselves.
Basic knowledge of how and when to de-escalate and mediate situations.
Ability to develop a working knowledge of my program and internal Avivo resources.
Having a flexible mindset to deal with various situations.
