Responsibilities



The Partial Hospitalization Program (PHP) Social Worker provides services within the PHP service line for individuals with psychiatric needs. The Social Worker is based out of our locations, in person. The Social Worker focuses on facilitating admissions, psychoeducation groups, treatment planning, discharge planning, care coordination, communication with the patient's support system, community providers, and others involved in the care of patients. Social Workers collaborate daily with psychiatric medical providers and therapists on the patient's primary Treatment Team to advance the plan of care within the average patient length of stay.
Hours for this role are Monday - Friday, 8a-430p
Responsibilities and Accountabilities:
Accountable to have knowledge and understanding of Social Work practices and principles
Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groups
Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
Responsible for communicating with patients and families to keep them involved in treatment plan decisions
Coordinates care with outpatient providers upon admission and discharge
Responsible for facilitating weekly treatment team meetings and creating master treatment plans/updating treatment plans as agreed upon by the treatment team
Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
Communicate clearly and effectively with clinical team members and Directors
Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department






Qualifications



Bachelor's Degree in Social Work required, Master's Degree in Social Work preferred, with licensure in the State of Minnesota required.
Experience in healthcare setting preferred
Experience using electronic health records preferred
Proficient in Microsoft Office Products including Word and Outlook
Able to effectively and efficiently communicate in written and verbal form
Ability to smoothly and rapidly shift between dissimilar tasks
Ability to communicate well both verbally and in writing
Ability to respond appropraitely in times of mental health crisis
