JOB REQUIREMENTS: We Are Hiring:Clinical Social Worker Hospice - Patient

Details:two days one week and three days the following week- Will
provide care for Home Hospice and Hospice in the Hospital patients M-F

education and management, injections and intravenous therapy, wound

more.Physical, occupational and speech therapists address home safety
concerns, strengthening and rehabilitation needs.Our licensed social
workers offer assistance with community resources, financial assistance
and supportive care and counseling to patients and families. What You
Will Do: Responsible for the timely effective and efficient
identification and counseling of patient(s) and their significant
other(s) who require psycho-social, emotional and financial counseling
in his or her assigned medical center area(s). This worker performs a
variety of social work functions in accordance with the accepted
principals and standards of the Social Work Practice What You Will Need:
