Avivo Chemical Mental Health Division is made up of two service Areas; Integrated Chemical Health/245G Substance Use Disorder programs and Community Chemical and Mental Health programs which provide a continuum of integrated services, including access, clinical treatment, care-coordination, and recovery support. We provide services that are innovative, person centered, strength based, and outcome driven. We strive for the highest quality of services and standards of care using culturally responsive harm reduction and evidence-based practices in a trauma informed environment.
The Community Intake Coordinator will coordinate and complete intakes for all community chemical mental health programs, assist in coordinating and completing intakes for community programs under the division of CCMH, and assist in client account setup for care coordination, case management, and other Avivo programs. This position will also follow up with referral and funding sources prior to, during, and after client admissions.
PRIMARY RESPONSIBILITIES
Demonstrate commitment to SAMSHA's definition of recovery and recovery principles: "A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential." www.samhsa.gov Serve as initial point of contact for internal and external referrals.
Screen and determine appropriateness for CCMH programs.
Ensure proper opening of new clients into TCM program/CSP/and MHC and with county, completing all necessary paperwork and data entry needed.
Coordinate with CCMH Program Managers and Directors for placement into appropriate CCMH teams.
Coordinate with Mental Health Clinic to ensure appropriate DAs are in place for placement into TCM, CSP, and/or MHC.
Evaluate, change, and further specify recovery goals on a continuing basis with each client.
Complete intake process for MHC, CSP, and One TCM programs.
*Monitor and manage the waitlist for MHC; coordinate with TCM data systems coordinator, CSP manager, Allina/SITT director to ensure smooth transition and entry into services among and within these Avivo programs and more.
Provide appropriate referrals to community resources and arrange transportation to appointments as needed.
Communicate with other community professionals and providers to ensure that services for clients are coordinated.
Maintain client files and program records, court reports and develop reports and billing statements as required.
Travel to different Avivo sites as most of the direct client contact occurs in the community programs, as well as including in participant homes, hospitals, encampments, jail.
Qualifications



Education and Experience
Bachelor's Degree in a Human Services related field or High School degree and 3 years of experience.
One or more years of experience working with single adults experiencing mental illness and/or homelessness or similar experience.
Lived experience will be valued and considered as part of the experience requirement.
