As the Community Engagement and Partnerships (CEP) Manager, you will oversee the daily administrative, external affair, community engagement, and strategic partnership functions related to the Social Care team’s development and management of a preferred network of Community Based Organizations (CBOs) and social care resources to serve and align enterprise wide efforts related to Social Drivers of Health (SDOH) and Social Care. You will work closely with the Ambulatory Care Management (ACM) Social Care unit and the enterprise SocialDrivers of Health (SDOH) Program leadership and management to develop and design innovative approaches to growing resources, partnerships, external stakeholder activation, and maintenance of those assets to address SDOH needs amongUCLA Health patients. You will lead, facilitate, coordinate and assist initiative implementation in collaboration with cross-functional internal stakeholders, including marketing, community benefit, strategic business partnerships, and other community engagement offices within UCLAHealth. You will utilize and tailor best practices related to community participatory frameworks, health promotion, and use a diversity/equity/inclusion lens to meet the needs of the Social Care unit and to support relevant features of the SDOH Program. You will support the implementation of established and emerging innovative initiatives to population health. As needed, you will travel to CBOs, UCLA Health sites, and/or relevant conferences to augment the visibility, participation, training, and engagement with formal and potential partners. You will also be responsible for overseeing the implementation, updates, curated lists, and other relevant materials related to the community engagement collateral. In partnership with the Assistant Director of Social Care, you will ensure the proper contracts and Memorandum of Understanding (MOUs) are processed and terms are executed according to plan. In this role, you will collaborate and influence key stakeholders including CBO staff and leadership, physicians, office staff, social work clinical advisors, community health workers and other key stakeholders.
Salary Range: $84,300 – $166,700 Annual
Required:
- Master’s Degree Required – Background in Healthcare Administration, Business, Public Health or a closely related field
- Minimum 1 year of experience in developing and maintaining relationships with multiple organizations; preferably with University of California campus and/or Medical Center
- Prior experience with community engagement, health promotion, and/or establishing and maintaining cross organizational partnerships
- Computer skills in with Adobe and Microsoft products (e.g., Word, Excel, PowerPoint, Teams and Outlook)
- Skill in analyzing information, problems, situations and procedures to define objectives, identify pattern and formulate conclusions
- Ability to creatively integrate competing demands of an outpatient setting into a productive working environment
- Ability to interact diplomatically and empathetically with a patient population in varying degrees of health and effective communication with staff, faculty and the general public in conveying information to varying organizational levels
- Proven experience communicating complex information to physicians and leadership in a diplomatic and productive manner
- Skill and experience in developing Quality Improvement action plans, including conducting case studies, supporting external affairs marketing campaigns, organizational development and root cause analyses
- Interpersonal skills and ability to communicate effectively and diplomatically with patients, staff and physicians; give and receive information
- Ability to be flexible with working hours given adequate notice
- Excellent communication skills; verbal, written and interpersonal. Proficiency in documenting in project management or similar platforms
- Superior analytical and problem solving skills
- Strong leadership skills and independent decision making ability
Strongly Preferred:
- Proven understanding of the social care, social drivers/determinants of health, and related payment models or literature
