Responsibilities:
Lead and inspire the health services team to deliver exceptional care and service.
Conduct initial assessments of prospective residents.
Ensure coordination of care with providers.
Implement effective systems, policies, and procedures.
Supervise and support RNs, LPNs, and unlicensed staff
Evaluate staff performance and provide constructive feedback.
Foster a positive team environment and promote a healthy workplace culture.
Qualifications:
Current RN license in the State of Minnesota
Supervisory experience in home health, assisted living, or long-term care
Strong organizational skills with a proactive approach to work
