We at Fairview Health Services are looking for a dedicated Casual Pediatric Clinical Treatment Coordinator! Are you looking to join a team to provide assessments and assist patients in getting levels of care? Join our team.

The Pediatric Clinical Treatment Coordinator (CTC) will partner with the physician and interdisciplinary team to navigate the patient/family through treatment episode, including assessment, treatment planning, discharge planning and community re-entry. This includes, but is not limited to: managing patient/family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and supervising and managing patient length of stay and avoidable days

Demonstrates clinical competence in the treatment planning process.

  • Coordinates treatment planning and leads management of each case.

  • Coordinates/facilitates the development of treatment plan by partnering with the physician and collaborates with patient, family and interdisciplinary team.

  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

  • Implements, reassesses and adjust treatment plan as vital.

Demonstrates clinical competence to assess individual patient needs.

  • Completes an interdisciplinary assessment tool with each patient.

  • Updates interdisciplinary assessments' tool each week.

Demonstrates clinical proficiency to partner with inter-disciplinary team in the treatment planning process.

  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

  • Implements, reassesses and adjusts treatment plan as vital.

  • Communicates frequently and optimally with interdisciplinary team to ensure continuity and coordination of care.

  • Documentation is detailed, timely, legible and clinically appropriate and meets regulatory requirements.

Demonstrates competence in developing a clinically relevant transition/discharge plan.

  • Partners with physician and family to provide leadership to the inter-disciplinary team

  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.

Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups.

  • Provides individual, couples and family assessment/therapy where indicated as part of individual treatment.

  • Documents thoroughly, timely, legible and clinically appropriate.

Demonstrates knowledge of utilization review and collaborates with the Utilization Management Specialists to insure the ongoing, comprehensive monitoring and evaluation of patient care, by concurrent and retrospective review to assure appropriate reimbursement.

Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are improved.

Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities:

  • Utilizing evidence based policies in clinical practice

  • Attends and participates in learning opportunities about evidence based practice

  • Membership in hospital or unit based practice council/research council

  • Evidence based practice project (e.g. policy development or poster presentation)

  • Journal club, journal article critique and circulation

  • Literature review

  • Development of standard/pathway/guideline for patient care

  • Presentation

  • Research study

  • Responds and participates in Behavior Control Situations (BCS) and CPR.

Qualifications

Qualifications :

  • One of the following current Minnesota licenses: Licensed Independent Social Worker (LISW), Licensed Independent Clinical Social Worker (LICSW), Licensed Graduate Social Worker (LGSW), Licensed Psychologist (LP), Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Associate Marriage and Family Therapist (LAMFT), or Licensed Marriage and Family Therapist (LMFT)

  • Master’s degree in psychology, counseling, social work, marriage and family therapy or a related field.

  • Two years’ therapy, case management, or assessment experience, including providing face to face therapeutic/clinical services to adult, or child/adolescent clients.

  • Basic Life Support Certification prior to end of orientation.

Preferred:

  • Master’s Degree in Social Work

  • Licensed Independent Social Worker (LISW)
