The Clinical Pharmacy Technician plays an important role in protecting our patients from medication errors caused by inaccurate medication histories. Under the supervision of a pharmacist, the Clinical Pharmacy Technician will function in the critical role of obtaining an accurate medication history. This will include interviewing patients, utilizing software and contacting their outpatient pharmacies/facilities. The home medication list compiled by the Clinical Pharmacy Technician will have any discrepancies reconciled by a registered pharmacist to ensure patient safety and optimize pharmacotherapy.
This is a temporary position as part of a pilot for the network that will run through the end of 2024 with potential for continued employment thereafter.
A day in the life of a Clinical Pharmacy Technician (Emergency Department) at Hackensack Meridian Health includes:
1. Obtains accurate medication histories from patients in the Emergency Department or inpatient units as necessary
2. Interviews patients/patient's caregivers to obtain most precise data about the patients medications and supplements (e.g. name of medication, dose, frequency, route, last dose taken, last time refilled)
3. Calls patient's outpatient pharmacy and or healthcare facility to obtain the best possible medication history (as above)
4. Utilizes electronic health record to collect patient related information and document work done as necessary
5. Communicates all medication discrepancies or complicated medication regimens to registered pharmacist
6. Utilizes available resources to communicate with a translator if necessary
7. Communicates with other healthcare professionals as necessary
8. Demonstrates an understanding of and adheres to all medication use and pharmacy policies and procedures.
9. Performs other pharmacy technician duties as assigned.
10. If Outpatient Pharmacy Technician is qualified, enter prescriptions into the order entry system.
11. Other duties and/or projects as assigned.
12. Adheres to HMH Organizational competencies and standards of behavior.
