Job Description

 
JOB REQUIREMENTS: Essential Duties and Responsibilities include the
following. Other duties may be assigned. Promotes and maintains the
health of individuals, families, and the community through teaching,
counseling, and initiation of appropriate preventive, therapeutic, and
rehabilitative measures. Provides nursing services in client homes,
clinics, schools, and industries. Assesses public health needs of the
community and utilizes the nursing process to promote and improve the
health of individuals, families, and the community. Plans and manages a
caseload and provides nursing services, including immunizations, to
individuals and groups in a defined geographic area. Conducts and
participates in epidemiological investigations to prevent the spread of
communicable disease according to Wisconsin statutes and agency policy.
Provides health education and communicable disease follow up to
individuals/families and community groups. Coordinates services with
other professionals and community partners to achieve a holistic
approach to consumer needs. Collaborates with community partners to
participate in community health improvement projects and facilitate
conversations to achieve common goals. Collects, analyzes, interprets,
and disseminates data from various sources. Prepares and submits records
and reports according to agency policy. Completes accurate, detailed
charting as appropriate, using both paper and electronic methods, and
maintain confidentiality at all levels. Assumes administrative
responsibilities as designated by the Health Division Manager or the
Public Health Nursing Supervisor. Participates in developing and
implementing a community preparedness plan. Participates in quality
improvement, performance management, and strategic planning activities.
Assumes responsibility for self-development and participates in
appropriate continuing education activities in order to maintain
competence within the scope of own practice. May be assigned to perform
on-call duty. It is unlikely an employee will perform all the duties
listed on a regular basis, nor is the list exhaustive in the sense it
covers all the duties an employee may be required to perform. The

Responsibilities This job has no supervisory responsibilities. Provides
mentorship to nursing students. \*\*\*\*\* OTHER EXPERIENCE AND
QUALIFICATIONS: Comprehensive knowledge of the nursing process
(assessment, planning, implementation, evaluation), and of the legal
scope and skill in current nursing practice. Thorough knowledge of the
practice of Public Health nursing. Obtain knowledge of, and demonstrate
effective use of, community resources. Thorough knowledge of inter
professional relationships in the implementation of programs. Ability to
seek and utilize supervision/consultation effectively. Ability to apply
appropriate theory that is scientifically sound as basis for decisions
regarding nursing practice. Well organized and efficient in managing
