Job Description

 
PT. Community Habilitation Specialist. Olean Benefits available after 90 days for Full Time Responsible for assisting individuals to become more independent when performing activities of daily living to achieve their valued outcomes. Habilitation services are designed to develop skills to increase an individual's level of independence. Services are primarily community based and revolve around volunteer and recreational opportunities in Allegany, Cattaraugus, and Chautauqua Counties. Principal Activities Serving individuals to gain experiences within their community or home base to accomplish goals set in Life Plan. Ensure effective instruction and safety of individuals following the Staff Action Plan. Ensure documentation is kept in compliance and submitted in a timely manner Work independently and be a team player. Complete all required training. All other duties assigned by Executive Director. Job Qualification High School Diploma or GED. Desire and compassion to work with diverse population. A valid driver's license with a clean driving record is a MUST. Maintain current auto insurance. Communicates well verbally and in writing. Proficient in Microsoft Office. Maintain a clean and safe working environment. Must be able to comply with HIPAA requirements Must have good listening skills with creativity. Experience in a Human Service environment. Effectively manages multiple tasks and priorities Understands and performs job results in a non-judgmental manner. Ability to interpret policies, procedures, and other written materials. Accepts supervision and follows through with supervisor recommendations. Treats colleagues, staff, and program participants with respect. Must be able to comply with HIPAA requirements.
