1.) Assessments, Referrals, Treatment Planning and Care Coordination:
- Identify depression using the Patient Health Questionnaire-9 (PHQ-9)
- Identify alcohol/substance use disorders using the Modified Simple Screening Instrument for Substance Abuse (MSSI-SA)
- Identify anxiety using the GAD-7
- Work alongside support staff, including social work and case management, across the Aging Services network.
- Refer clients to a qualified mental health professional within five business days of completing the assessment.
- Coordinate and run Group Supportive Counseling and Mental Health Education sessions monthly.
- Follow up on mental health and substance use needs, treatment planning, and coordination monthly.
- Document all assessments, referrals, and client interactions in STARS and the database.
2.) Case Management
- Provide Case Management and Case Assistance
- Develop care plans to promote client self-determination; assist seniors with staying in the community; and promote healthy and independent aging.
- Prepares documentation (assessments, case logs, case notes, etc.) as stipulated by contractual and agency standards and procedures.
- May cover for the Clinical Manager in his/her absence.
- May be assigned to handle more involved cases.
Qualifications/Educational Requirements
- LMSW Degree with a minimum of two years experience
- Bi-lingual English/Spanish required
- Experience with Geriatric population preferred
- Experience with Mental Illness and Substance use preferred
- Experience with assessments required
- Experience with care coordination required
Computer Skills: To perform this job successfully, an individual should be:
- Able to work in the Microsoft Office Suite, should especially be proficient in Word, Outlook, and Excel
Physical Requirements
- Visit multiple sites within the Aging Network
- Home visits
