Job Summary:  Center for Human Development, Inc. is seeking a part-time (30 hours) Behavioral Health Nurse Case Manager to dispense and deliver psychiatric medications to SPMI individuals or consumers in acute crisis. Consumers may have complex medical and psychiatric conditions. Work closely with clinical staff, including prescribers, to monitor mental status and observe physical, emotional, and general status of consumers at each visit. The NCM will take vital signs at least monthly/ more often for consumers who have a blood pressure or other medical problem and report any changes or abnormal findings to MD, PMHNP. NCM is also expected to work independently in a self-motivated manner. The BHT Nurse Case Manager works under the direction of the Associate Medical Directors.



The Requirements:

Current RN license required.
Experience in psychiatric/mental health field preferred.
Able to work with a variety of clients and diagnostic symptoms.
Familiar with psych meds and side effects.
Familiar with diagnoses.
Willing to seek consultation/supervision.
Must possess a valid Oregon Driver's license and be insurable for client service purposes, travel between business offices, and to attend required meeting and/or trainings.
Must pass all criminal history check requirements as required by ORS 536-181.537 and in accordance with OAR 407-007-0200-407-007-0370 and drug screening.


Essential Job Functions:

Order prescription refills from pharmacies.
Maintain professional relationships with all relevant pharmacists and keep them informed of changes in medications, problems with meds, etc.
Pick up medications for clients as needed
Set up medications in 'pill minders' for consumers who receive medication delivery services
May give injections when prescribed by LMP
Transport consumers as needed (lab work, MD appointments, other appointments)
Teach consumers about medications and other health issues
Reinforce/follow ISSP treatment plans of CHD staff and orders from other medical care providers
Pick up relevant equipment and supplies
Make home visits
Maintain appropriate professional communication with medical care providers and provide current updates of consumer's condition to medical care provider appointment whenever possible.
Write appropriate progress notes for each encounter with a consumer.
Must maintain punctual and regular work attendance.

Salary:   $4,907.50 to $7,099.30 per Month Depending on Experience, Depending on Qualifications
