High School Diploma or General Education Diploma (G.E.D).
Must successfully complete the online CHR Basic Training Course within first year of employment.

Physical Requirements:
Ability to lift at least 50 lbs.
May be exposed to communicable diseases and/or blood borne pathogens.
May require driving in sub-optimal weather and road conditions.
Must be able to work in adverse circumstances. Non-smoking environment.

Knowledge of commonly used concepts, practices and procedures within the medical field.

Able to work independently and establish work priorities.
Able to work very early mornings, late evenings and/or some Saturdays and holidays.

Must possess professional integrity.

Native American preference.

Job Description:
Community Health Representative
Division: Health Division
Reports to: CHR Program Manager
Worksite: Health Division
Work Hours: M-F, 8:00am - 4:30pm, or as needed.
LLBO Status: Non-Exempt
Safety Sensitive: Yes
License/Certification Requirements: Must have a valid MN Driver's License, current insurance or be insurable.

Summary: Provision of individual assessment, therapeutic and follow-up services.

Duties/Responsibilities:
Provide transportation to medical appointments for dialysis, cancer, elder, diabetic and handicapped clients.
Provide monitoring and assessment on a daily basis through home contacts in individual service areas. Provide follow-up care on medical referrals made by Public Health Nurses, hospital staff and other health professionals.
Make regular home visits to senior citizens and handicapped persons. Will provide homemaker services, assist with obtaining medication if needed and monitoring health and safety needs.
Assist clients in and out of the vehicle when needed.
Will participate in training opportunities on health promotion/disease prevention issues in areas of general health care, maternal child health, environmental health care and AIDS.
Maintain a professional attitude towards the division, co-workers, other LLBO employees, vendors and staff. Willing to be trained using The Lift transport van, and how to safely transport clients to and from appointments. Reconcile mileage logs, trip sheets, vehicle check lists, on a weekly basis and turn them into the CHR Program Manager.
Will maintain compliance with HIPPA and OSHA.
Will adhere to JCAHO (Joint Commission on Accreditation for Healthcare Organizations) requirements.
Represent the Leech Lake Band of Ojibwe and Health Division in a positive and professional manner.
Adhere to policies governing staff conduct.
Attend and or conduct all mandatory meetings and trainings as required.
Maintain confidentiality and professional ethics at all times.
Ability to travel in and out of the state on a possibly regular basis.
Understanding and sensitivity to Native American culture. All other related duties as assigned.
