Job Description

DIAGNOSITC ASSESSOR - FULL TIME - ALLEN COUNTY

Coleman Health Services is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a Diagnostic Assessor in Lima, OH. The Diagnostic Assessor will provide quality assurance and regulatory compliance to client files, complete DA, and initial treatment plans for the walk-in clinic.

Diagnostic Assessor Qualifications:

Bachelor’s Degree and LPC, LSW or Psych Assistant
Excellent skills in diagnostic assessments
Experience working with SPMI, SMD and SUD adult population
Must be willing to obtain or already have the following: an NPI number; and an Ohio Medicaid number.

Preferred Clinicians:

LPCC, LISW with LCDC
