Relief Staff (LPN)
Description

Duties and Responsibilities

  • Is available to work, at a minimum, for each of the agency’s programs in the employee’s

home county.

  • Completes at least one relief shift per calendar quarter.
  • Reviews and signs-off on the job description(s) for which coverage is requested.

Performs duties of the position(s) as assigned.

  • Maintains required credentials for the job description(s) for which coverage is requested.
  • Is responsible for reviewing agency and program policies and procedures.
  • Signs-off annually on required documentation (e.g. code of conduct, professionalism

policy)

  • Participates in required agency trainings.
  • Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and

interactions with fellow staff, residents, and external partners.

  • Maintains compliance with all required rules and regulations including Federal

Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.


Requirements

One year’s experience practicing as a Licensed Practical Nurse.

• Requires an unrestricted New York State License or Certification: Licensed Practical Nurse, CPR and First Aid Certified.

• Must possess a New York State driver’s license with an acceptable Motor Vehicle Record.

Salary Description
$27.00/Hour
