JOB SUMMARY:

Licensed Practical Nurse or Licensed Vocational Nurse

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

• Assesses, plans, and evaluates medical needs of youth using evidence-based practices.
• Supervises and delegates tasks to Medical Coordinators to ensure timely execution of physician’s orders including the self-administration of prescribed and over-the-counter medication.
• Creates a monthly department schedule to ensure medical services 365/24/7.
• Completes on-the-job training with new Medical Coordinators.
• Completes 30-day and annual evaluations for employees reporting successes and challenges.
• Adheres to policies and procedures for the storage of medication according to relevant state and federal standards.
• Conducts quality assurance checks on medication logs (medication administration records (MARS), medical charts/files, documentation and reports any findings to the Program Director timely.
• Provides support to the Specialized Intake Team and assists in assessing and addressing each child’s medical/healthcare needs upon intake. This will include interviewing youth using a child-friendly and trauma-informed approach to gather a comprehensive medical history, conduct/facilitate COVID testing, and obtain youth’s vital signs (blood pressure check, pulse, breathing rate, temperature check, etc.), including height and weight check and checking for any irregularities on the youth’s body (rash, blisters, etc.)
• Reports any medical conditions identified for youth and report to Program Director and seek appropriate medical care. If necessary, may need to contact 911 or coordinate transportation and escort services for transportation to a hospital.
• Ensures that all information is documented in the youth’s medical chart and/or medication administration record, in the UC portal and alerting appropriate staff of any concerns that require immediate or follow-up medical care. This may require completing Incident Reports and/or Significant Incident Reports for submission to program management staff, state, and federal representatives.
• Maintains regular contact with other department leads to provide updates on any special medical needs regarding youth.
• Ensures that High-Risk forms are completed timely to ensure that all departments are aware of any special medical needs being provided to youth.
• Maintains the correct ratio of staff to children while medical care is being provided to youth. Ensuring that supervision is based on children’s needs and adhering to any safety plans.
• Conducts appropriate supervision of youth based on each child’s need and required staff-to-child ratios. Must stay up to date on any high-risk behaviors that may require increased supervision of youth during medical care or medical appointments.
• May need to respond to emergency events to ensure the safety and security of the youth in care.
• Follows the approved behavioral management system when needing to address/interact with youth.
• Must never take a youth acting out or escalating negative behavior personally.
• Completes all required documentation daily and timely based on required timeframes as set by relevant state and federal (ORR) contract requirements.
• Completes comprehensive Incident Reports (IRs). IR must capture a youth’s behavior prior to an incident, what lead up to the incident, all circumstances of the incident observed/heard, interventions made during and after the incident, contacts made and resolutions.
• Reports headcounts and the movement of the children on an ongoing basis.
• Needs to enter daily medical progress notes, any medications administered to youth and any assessments completed during their shift into UC portal.
• Required to arrive at the shelter on time, review/ receive any updates, changes in procedures or supervision of youth, and always arrive prepared to work regularly and consistently.
• Assists with the maintenance and upkeep of surrounding workstations as needed. Ensure that all medical/treatment service areas (office(s)) are properly sanitized on an ongoing basis to prevent the spread of infectious diseases.
• Ensures that there are no safety hazards for youth or staff whether indoors or outdoors and must react proactively to prevent immediate harm.
• Ensures that administrative duties (filing, preparing casefiles/medical charts, etc..) of the clinic or designated site(s) are completed, in accordance with relevant state licensing and federal contract requirements.
• Assists medical provider during examinations as needed.
• Communicates effectively with healthcare providers regarding care for youth.
• Conducts/Maintain an inventory of all medications/supplies, ensure medications/supplies are within date and properly stored according to relevant state and federal (ORR) contract requirements.
• Properly dispose of all unused and expired medications according to policies and procedures.
• Complete/Assist in conducting daily medical audits ensuring proper documentation is

maintained.

• Reports any medication errors through internal reporting requirements and create a plan of action to correct the error and prevent future errors.
• Carry out all treatment orders and administer prescription and over the counter medication to youth as directed by medical provider, ensure orders are implemented timely.
• Completes on-the-job training with new Medical Coordinators.
• Reviews and approved work schedules and requests for time off.
• Attends program and departmental meetings as needed.
• Ensure adherence with CDC guidelines related to communicable diseases to help prevent the spread of COVID-19, Flu or other illnesses.
• Seeks appropriate guidance from supervisor as needed to ensure medical care and staff to child ratios are always adhered to.
• Prioritizes tasks daily to ensure appropriate execution and meeting timeframes.
• Performs other duties as required or assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS:

• Registered Nurse from an accredited college or university is preferred with 2 years of experience working in a clinic or hospital setting, may use internship or volunteer hours, or
• Licensed Vocational Nurse/Licensed Practical Nurse with 3 years of experience in a clinic or hospital setting.
• 1 year of experience working with children or youth is preferred.
• Proven experience in a supervisory position preferred.
• Current basic life support certification, PALS, and ACLS preferred.
• Show a history of organizational skills and an ability to communicate accurately and effectively with multiple members of a team.
• Well organized, highly motivated, excellent communication skills, leadership skills that also display integrity.
• Understanding of Health Care Service procedures.
• Proficiency in Microsoft Office is preferred.
• The ability to type and use a tablet/iPad, laptop, or desktop is needed to complete daily tasks.
• Display the ability to understand and retain information relayed in training, daily shift change, and departmental or program meetings.
• Able to work cooperatively with multiple members of a Team including program staff, state licensing personnel, ORR/Federal Field Staff, and other office personnel.
