Job Summary: |
This is a 1.0 Full Time position with the Tri-County 911 Service Coordination Program, aka TC911. TC911 is a program focused on residents who use emergency medical services (EMS) frequently, when other non-emergency resources better meet their needs. The EMS Clinical Services Specialist is part of a small team of licensed clinicians who outreach to, and engage with, clients and existing providers to assess needs, link to the most appropriate services and resources, and coordinate care across varied systems. TC911 serves over 500 individuals annually with intensive, yet short term interventions. Clinicians are employed to improve individual care and quality of life as well as reduce impacts to emergency medical systems, including 911 dispatch centers, medical first responders, and emergency departments.
This position is administratively managed and housed in the Multnomah County EMS Program. The position does involve providing service in and travel throughout Multnomah, Clackamas and Washington counties. The EMS office provides pre-hospital emergency medical care to the residents and visitors of Multnomah County, regulates and coordinates all pre-hospital EMS care in the County, and ensures optimal patient care through quality improvement activities.
As the EMS Clinical Services Specialist (LCSW OR LPC), you will:
Work with, and on behalf of, individuals who frequently interact with emergency medical services (EMS) and emergency departments (EDs)
Conduct phone, in -person and field based services in Multnomah, Clackamas and Washington Counties
Outreach in community settings, such as local EDs, jails, homeless shelters, medical clinics, and private homes to facilitate engagement and improve care coordination
Assess and address individual and system needs, including but not limited to, conducting biopsychosocial assessments, providing crisis intervention, clinical case consultation, multi-agency care coordination and collaboration
Manage complex and multifactorial issues (e.g. mental health, substance use, homelessness, cognition impairments, safety) which impact inappropriate or overuse of 911 and emergency medical services
Collaborate with regional county agencies, health and behavioral health providers, social service, safety net and other community partners to connect individuals to the most appropriate, non-emergency systems of care
Determine and apply culturally relevant, individual, community and systems-level clinical approaches to improve quality of life and care
Work with both clinical autonomy and within a team framework
Present information to system, agency and other stakeholders
Utilize good clinical and professional judgment in client engagement, relationship building; collaboration, and applying best practices
Assist with planning, coordination and implementation of the "911 service coordination program of EMS
